How to Create a New Template

1. Under the Template tab, click the 'New' button. You can select create new or upload a template if you are updating a current form.

2. Name the form templates to match the naming convention standards (see Forms Specific Requirements document).

  1. Add template description (See Forms Specific Requirements document).
    1. Name: StateAbbrFormNameYYYYMMDD (ex. CAUM_UIM SELECTION FORM20180801)
    2. Description: Uninsured and Underinsured motorists Protection (UM/UIM) Selection Form for California

3. Upload Form to build template over.

4. Set Signing Order. If the form will need to go to a third party, you will need to add a second recipient, otherwise the only recipient would be 'Insured'

  1. Scroll down to Advanced Options and select 'Edit'
    1. Under 'Sender Settings' uncheck 'Affix envelope to top left of every document page'
    2. Click 'Save'

    Third Party Instructions

    1. Creating text boxes that will be used to send to third party signer. Although the insured will complete this, select the person who will receive the document from the drop down menu.

    2. For the text boxes that indicate what to enter in the adjcent boxes, check Read Only under Recipient and Recipients Can Collaborate under Collaboration. Need to add data label (see Forms Specific Requirements document).




    Detailed Instructions for the Bypass

    1. After adding all the other elements, then add a radio button from the Standard Fields.
    2. Give the radio element a Group Label

    3. Enter values for each of the Radio Button options

    4. Add the Bypass Note
    5. Add Custom Text: Bypass_No and Bypass_Yes

    6. Add the Bypass_No_Note and the Bypass_Yes_Note

    7. Tip! Offset the notes from the text so that it is easier to select and move each pair. After the conditional logic is in place, you can move these back into their correct positions.

    8. Select the Bypass radio button and then click on Create Rule under Conditional logic.

    9. Select the trigger field from the dropdown.

    10. Select Choose to Not Sign from the drop down menu and then on the form select the Bypass Note and the Bypass Yes text.

    11. Click Done and the Conditional Logic element will show:

    12. Note that the Bypass Note shows that the field is now conditional:


    13. Select the Bypass Radio element and then select CREATE RULE in the Conditional Logic section. DO NOT select edit because we are not editing the same rule, but are instead creating a new one.
    14. Select Agree to Sign from the drop down menu and then sellect all the fields that you want to be visible if this option is selected.
